Has anyone developed any model prescriptions for prescribers to refer that cover installments, pharmacy opening, dispensing in advance in daily containers etc?

It's in the clinical guidelines page 106-107.

as simon says. I am pleased to see it says it is possible to state "dispense on mondays, wednesdays and fridays" without specifying amounts for each day. Occasionally pharmacists object to this, but on Emis (commonly used primary care software) it is a lot easier than working out all the amounts.

some software has options for adding wording about missed doses, and take home doses, but in EMIS we have to free text all these things, so i have a nice stamp (in an extremely small font) for the "missed installment " wording which i use for people who tend to miss the odd pickup at times when life intervenes.

If I understand the point here, the reason we object is that our professional code of practice tells us to. On p29 of Medicines, Ethics and Practice, it says "The Home Office has confirmed that an instalment must have both a dose and an instalment amount on the prescription (ie they both have to be specified separately)."

I didn't use to have a problem so long as the number of days is clear ("600ml, dispense on Mondays, Wednesdays and Fridays" is fine if I know how many of each are meant to be included) but now we have this instruction, we're bound to follow it.

Hi
I have been on the Home Office website about this
And the advice is that the installment amount must be specified
The confusion is what we call an installment
The Home Office classes an installment as the dose amount, in your case, Judith, you are classing the installment as the amount they are picking up Monday Wed and Friday
I do agree that the Orange Guidelines do not state this
But I spent 3hours trying to get to the bottom of this on the HO website..and I am afraid this is what I have found
And I have probably made it as clear as mud, sorry

I'm currently working to Judiths model. No one has complained, no one has threatened to prosecute me, we're paid for the prescriptions and I've the clinical guidelines (and my reading of the legislation) to at least suggest that the position is legal.

I'll also ask for clarification.

Rob how about this
'xml daily supervised. Dispense take home days on friday for w/ends in separate daily dose containers in advance. Dispense for b/hols on last day before closure'
Above for Methadone

yml daily supervised. Dispense take home doses on friday for w/ends. Dispense for b/hols on last day before closure.

Above for Buprenorphine

The wording on last day before closure means some people will have to attend if the Pharmacy is open, but you won't know exactly which pharmacies will be open bank hols.
Hope that helps.
