I've discovered the Drug & Alcohol Action Team are requiring nurses, employed to assist practices involved in substitute prescribing, to complete the TOP forms for all patients quarterly.
My concern is this form includes patient name/dob/sex which must be a clear breach of Caldicott guidelines.
It also appears that no form of informed consent is obtained and as far as I'm aware practices are not aware this is happening as the nurses just get on and do it!
Has anyone else come across this?

Thanks Simon,
glad to find others think asking patients clinically driven questions is a useful exercise, but collecting crime statistics is a joke (both ethically and in terms of veracity).
Think I'll run the issue of informed consent and Caldicott via our LMC.
