I am doing an audit into DNA rates at my GP-lead shared care clinic for clients already established on methadone or subutex. I am difficulty finding an external standard for DNA rates- has anyone done a similar audit or can tell me what their clinic's DNA rates are?

Can't answer the other questions but at our clinic the patient gets the prescription at the appointment so DNA rates are quite low.

We have an audit of review by prescriber and keyworker but it is a snapshot rather than a genreal service overview.
I am not aware of a national standard, except to say that in the Orange Guidelines the review rate for clients on opiate scripts should be around once every 3 months. It probably depends on what you are asking of the client and what risks you are aware of. However if you don't see clients, the danger for us is that we don't know the current situation.
I would agree with Simon that linking a script to a clinic appt usually sends a message ( correctly ) that review is necessary at reasonable intervals, all of which has been discussed in different threads. Being able to hold onto the script till the client arrives for it ( or contact you ) would be the way to ensure it works.

Agree re tying appointment to script etc and then generally low in general practice but has been really high in some services

+ interesting paper in August clinical update re DNA rates (not primary care) but interesting and also worth looking at recent publication from NTA entitled 'towards successful treatment completion: A good practice guide'
