Hi there.

I am looking for a CIWA / CIWA-a sheet that we could use locally for symptom guided withdrawal - preferably with guidance as to recommended chlordiazepoxide doses.

Things i have found on the net have either had a lot of branding over them or copyright issues are not clear. i did find a good CIWA-ar example but I don't like the format of it (would need a seperate form for each assessment - i am looking for a table version that could be used on a ward (if that makes sense).

What a good idea, I've used it in hosp from time to time, and just gave the nurses one sheet, so not sure how they went about using it. However whilst searching, I came across a great website: http://www.ciwa-ar.com it has training videos that show you how to do the scoring (and one showing a doctor taking a bad history!)

I suspect its not a good idea to be too prescriptive about linking doses to symptoms, there will be variable susceptibility to chlordiazepoxide and some will have a cross tolerance from use of other benzodiazepines.
